Clinical Indications

For patients who may require a blood transfusion

Request Form

Blood Transfusion form (Pink) (Code: BAT451)


On request.

Specific Criteria

Relative to previous transfusion history, please call laboratory for advice.

Turnaround Time

Same day. If irregular red cell antibodies detected repeat samples may need to be sent to NBS referral lab. Turnaround on referred tests can be 7-10 days. If required urgently call the laboratory for advice.


Whole Blood


1 x 6mL (If Known red cell antibodies 3 x 6ml EDTA)


Purple top (EDTA) tube.


Samples and forms must be labelled in full  as per the sample acceptance policy and should be transported to laboratory immediately.
The Blood Transfusion Laboratory has a Zero Tolerance Policy with regards to mis / incomplete labelling. All sections of the Form & Sample need to be accurately completed with the required available information.

Staff are reminded that the "ZERO TOLERANCE" policy will be applied where the labelling of blood samples and / or the information provided on the blood transfusion request form are incorrect.
This is to avoid near miss and avoidable errors in the transfusion process

Lab. Handling

Sample should be processed as soon as possible on receipt.

Causes for Rejection

Unlabelled or incorrectly labelled samples / forms click for guide.
Insufficient sample volume. Poor sample quality due to deterioration, haemolysis, presence of clot.



Incorrect labelling of tube / form will delay testing and subsequent blood  issue.
Some red cell antibodies may cause delay in providing compatible blood.
Recent massive transfusions of a differing ABO / Rh may mask the patients ABO / Rh group.
